First thing’s first, let’s put aside the religious connotations of prayer (you don’t need to be religious to pray). Our emotional, mental, or physical health, often impacted by familial, societal, or cultural factors, can contribute to this condition. Furthermore, the lack of mindful, self-reflective practices can also trigger this condition. While a sense of spiritual emptiness is an experience familiar to many people, it is extremely common for those who struggle with addiction. While the absence of spirituality in no way causes addiction, it is generally accepted that addiction has a spiritual component.
Trauma and PTSD Symptoms: Does Spiritual Struggle Mediate the Link?
Additionally, lack of self-care and self-compassion, including poor eating habits, not getting enough rest, and neglecting basic needs are critical contributors. Spiritually, the lack of connection with nature, devoid of meaningful relationships, and a lack of faith can also contribute to spiritual malady. The spiritual malady is the result of my being out of order with my higher power who I choose to call God. I was the director in the drama of life and managing the world so I could get what I thought I needed to feel ok. This “fourth dimension”, which we find out in the 10th Step is the “world of the Spirit”, takes us beyond the physical, mental, and emotional dimensions of life — and eliminates the selfishness (ego) of the “spiritual malady.” The term “spiritual malady” does not mean that our “spirit” is sick. It simply means we are spiritually blocked off from the Power of God, which enables us to remain sober, happy, joyous, and free.

What is Spiritual Malady?
Some clients may benefit from treatments designed for spiritual struggles (e.g., Cole & Pargament, 1999; Murray-Swank & Pargament, 2005). Effectiveness of such treatments may lie in allowing victims to alter the meaning of their trauma in order to view the world, themselves, and a higher power in more benevolent and flexible ways. The subscales of spiritual struggle parallel not only negatively-valenced cognitive content, but also cognitive processes for reducing distress after trauma. Differences in the success of the process may explain the differences among subscales in their relations with PTSD symptoms. Negative religious reappraisals may represent over-accommodation (e.g., God is no longer powerful), assimilation (e.g., God is punishing me for something I did), or accommodation (e.g., evil forces cause negative events, but God and most others are benevolent). Distinguishing adaptive accommodation from maladaptive over-accommodation and assimilation may further explain spiritual struggle’s relationship to PTSD symptom development and maintenance.
We accumulate threat and trauma codes throughout a lifetime that bias the system towards threat and trigger threat physiology easily. Financial threat and injury have similar effects to physical threat and injury, too. What is A Spiritual Malady?
The other safety phenotype is one of relative immobilization where we relax, repair, recovery, digest, defecate, contemplate, daydream, sleep, dream, and experience spiritual connections. This is frequently referred to as “digest and rest” physiology although it too is much more than that. On page 62 the text explains that “Selfishness-self-centeredness! That, we think, is the root of our troubles.” This “SELFISHNESS-self-centeredness” (or the “ego”, as some people refer to it) drives us to respond to life situations with the above “symptoms” as well as disorders and addictions other than alcoholism.

Thankfully, the “spiritual malady” is no longer a “missing piece” of Step One for me.
